Waiver and Release of Liability

ACKNOWLEDGMENT OF RISK
I, the undersigned parent or legal guardian of the above-named participant, acknowledge that participation in athletic training, sports activities, conditioning programs, and related activities offered by Legend's Academy Youth Program involves inherent risks. These risks may include, but are not limited to, muscle strains, sprains, fractures, concussions, serious bodily injury, or other injuries that may occur during participation. I understand that these risks cannot be completely eliminated and voluntarily assume all risks associated with my child’s participation in the program.

RELEASE AND WAIVER OF LIABILITY
In consideration of my child being permitted to participate in activities with Legend's Academy Youth Program, I hereby release, waive, discharge, and hold harmless Legend's Academy, its owners, coaches, trainers, staff members, volunteers, affiliates, and facility partners from any and all liability, claims, demands, actions, or causes of action related to injury, illness, damage, or loss that may occur as a result of my child’s participation in the program. This release includes any claims resulting from negligence, except in cases of gross negligence or willful misconduct.

MEDICAL AUTHORIZATION
I certify that my child is physically able to participate in athletic activities. I understand that it is my responsibility to inform Legend's Academy of any medical conditions, injuries, or physical limitations that may affect my child's participation. In the event of an emergency, I authorize Legend's Academy staff to obtain medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses that may arise.

PHOTO AND MEDIA RELEASE
I grant permission for Legend's Academy to take photographs and videos of my child during participation in training sessions, practices, events, or activities. I understand that these images or videos may be used for promotional, educational, or social media purposes by Legend's Academy.

I have read and fully understand this Liability Waiver and Release. I agree to its terms voluntarily on behalf of myself and my minor child.
